Client Appointment Screening

Personal care services such as Massage & Bodywork are considered a close contact activity. Our staff and independent contractor therapists are currently symptom-free and to the best of their knowledge, have not been exposed to the virus.

By scheduling an appointment, you certify that you understood this is a public environment providing services to other clients who could be infected with or without their knowledge.

The Healing Arts and Massage School cannot guarantee an environment free of exposure to the COVID-19 virus.

All clients scheduling appointments with Students at The Healing Arts and Massage School or Independent Therapists/Businesses utilizing the treatment area must complete the appointment screening form below for each appointment. The screening forms are submitted within a 24 hour period prior to scheduled appointments. Your participation and cooperation will assist with promoting public health and safety during the current events associated with the COVID-19 pandemic in an effort to prevent the spread of the virus.

Licensed Massage & Bodywork Therapists, along with Student Massage Practitioners participating in clinic activities with the public complete a similar questionnaire and screening prior to entering the office.  Limited access to the school and 50% occupancy standards are maintained in accordance with CDC and Department of Health and Human Services guidelines or requirements.  All practitioners utilizing the office space as Students or Licensed Therapists will wear a face mask and maintain a high standard of sanitation for all activities.
