Continuing Education Self Screening
Personal care services and educational experiences such as Massage & Bodywork are considered a close contact activity. It is important for us to promote safety as Licensed Therapists, Students and Staff while engaging in educational activities in a public environment.
While participating in activities at the school you certify that you understand this is a public environment engaging with other members of the public who could be infected with or without their knowledge.
Due to the nature of participants entering the classroom/educational environment, The Healing Arts and Massage School cannot guarantee an environment free of exposure to the COVID-19 virus.
Practitioners participating in activities at the school must complete the self screening form below for each day prior to arriving at the school. The screening forms are submitted within a 12 hour period prior to arriving for a scheduled activity. Your participation and cooperation will assist with promoting public health and safety during the current events associated with the COVID-19 pandemic in an effort to prevent the spread of the virus.
Limited access to the school and 50% occupancy standards are maintained in accordance with CDC and Department of Health and Human Services guidelines or requirements. All practitioners utilizing the office space as Students or Licensed Therapists will wear a face mask when a minimum of 6 feet distance can not be maintained and maintain a high standard of sanitation for all activities.
